Excuse me, 123, what is "請假單"? You hand this in to the authority after your sick leave, or is it a form you need to fill out before taking your leave?

"Absence Form":
(category examples) i) "absent from school" ==> "(Class/School) Absence Form" ii) "absent from work" due to pleasure trip" ==> "(Travel) Absence Form" iii) ..., etc.

In my school ,we have an absent form. No matter why you are absent, all you need to do is to fill out the form and on the form , there's a blank which you can fill in the reason why you are absent. That's why i don't what travel absent form is. Lynn
